You are required to upload a Letter of Support. If you are not able to provide a Letter of Support, you will need to complete the “Letter of Support Form”. This completed form will be attached to the CHE application and will serve as your Letter of Support.

Curriculum Vitae (CV)

You are required to upload a copy of your Curriculum Vitae (CV) including detailed position descriptions.

Degrees, Diplomas and Certificates

You are required to upload copies of your most relevant degrees, diplomas and/or certificates.

Foreign Degrees

Degrees must be from a recognized Canadian accredited university. Should you have a degree from a foreign university, you must provide proof of equivalence by a Canadian accredited university. Please consult the Canadian Information Center of International Credentials for further information at: http://www.cicic.ca/indexe.stm

Prior Learning Assessment Recognition (PLAR)
If you do not have a university degree but have five years of progressive experience in the Canadian health system. You are required to complete a PLAR. (available in the application)

The PLAR is a systemic process that evaluates, identifies, documents, assesses and formally recognizes the learning and experience obtained outside formal educational institutions. The procedure provides candidates with the opportunity to articulate and demonstrate their abilities in relation to the capabilities set out by the CHE program.
